Human pregnancy lasts approximately 9 months (~38-40 weeks), counted from the last menstrual period. It is divided into 3 trimesters of about 3 months each.

Maternal changes during pregnancy

Parturition — Childbirth
Parturition = the process of childbirth — the expulsion of the fully formed fetus from the mother's body at the end of pregnancy.
Trigger: the foetal-maternal feedback
Parturition is triggered by a complex hormonal feedback loop:
- Foetal ejection reflex (NCERT): signals from the fully developed foetus and the placenta induce mild uterine contractions.
- Maternal pituitary releases oxytocin — a powerful uterine contractor.
- Oxytocin causes uterine contractions (myometrium) → push baby down birth canal.
- Cervix dilates (assisted by relaxin, which softened it earlier).
- Baby is delivered through cervix → vagina → outside.
- Placenta is then delivered as the "afterbirth" (third stage of labour).

The positive feedback loop
Oxytocin's effect creates a positive feedback loop:

- Loop continues until baby is born
- After birth, the loop breaks (no more cervical stretch).
Stages of labour
- Stage 1: Dilation — cervix dilates to ~10 cm. Lasts ~6-12 hours (longest stage).
- Stage 2: Expulsion — baby is delivered. ~30 min - 2 hours.
- Stage 3: Afterbirth — placenta is delivered. ~30 min.

Lactation — Feeding the Newborn
After birth, the mother begins lactation — the secretion and release of milk from the mammary glands to feed the baby.
Hormones of lactation
Two key hormones (review from Section 6):
- Prolactin (anterior pituitary) → drives milk synthesis in alveoli.
- Oxytocin (posterior pituitary) → triggers milk ejection (let-down reflex) when baby suckles.
Milk types
Colostrum — the first milk
- Produced for first 2-3 days postpartum.
- Yellowish, thick consistency.
- Very rich in:
- Antibodies (IgA) — provide passive immunity to the newborn.
- Proteins, vitamins A, E, K.
- Critical for newborn defence against infections.

The suckling reflex:
- Baby suckles at the nipple.
- Nerve signals from nipple reach the hypothalamus.
- Hypothalamus signals the posterior pituitary to release oxytocin.
- Oxytocin causes myoepithelial cells around alveoli to contract.
- Milk is ejected from alveoli → ducts → ampulla → nipple.
- Baby gets milk.
This is called the "let-down reflex" and is essential for milk delivery.
What if a mother doesn't breastfeed?
Without suckling stimulation:
- Prolactin levels drop.
- Milk synthesis stops.
- Mammary glands gradually involute (return to non-pregnant state).
- Lactation ends within ~1-2 weeks.
Benefits of breastfeeding
NCERT emphasises:
- Best nutrition for the newborn.
- Antibody transfer (passive immunity).
- Mother-infant bonding.
- Protective effect against gastrointestinal infections, asthma, allergies in baby.
